OFFICE POLICIES AND EXPECTATIONS

Welcome to our hybrid pulmonary clinic, designed with convenience and efficiency in mind. We offer a virtual-first model supported by in-office visits only when medically necessary. Please review the following policies carefully before scheduling or confirming your appointment. These are non-negotiable and intended to ensure smooth, respectful care for all patients.

• All administrative tasks (scheduling, billing, referrals, form handling) are handled by virtual staff.
• In-office visits are for medical care only with Dr. Mkono.
• No billing, scheduling, authorizations, or staffing issues will be discussed during appointments.
• Contact our virtual office at 561‑777‑5308 for all non-medical matters.

📄 Referrals (Required for HMO / Plan-Based Care)
If your insurance requires a referral:
• It must be faxed to 561‑303‑2131 before your appointment is confirmed.
• Include: ICD‑10 diagnosis codes, CPT codes, Dr. Mkono’s name, pulmonary medicine as the specialty, visit type, and valid date range.
• While we can fax a request, you are responsible for following up with your provider to ensure timely delivery.
• Upon request (via registered portal access), we can provide a referral form for you to hand-deliver to your provider. We do not mail referrals.
• If delays occur, follow up with your PCP or insurer. Internal delays will be communicated when applicable.

💳 Financial Check‑In & Payment Policy
All required fees must be paid in advance to complete check-in, including:

• Deductibles
• Coinsurance
• Past-due balances
• Cancellation or no-show fees
• Form/documentation fees
• Estimated out-of-pocket costs

• Bring your insurance formulary to every appointment.
• Prescription coverage is determined by your insurance, not Dr. Mkono.
• Prior authorizations will not be processed unless the official formulary is provided.
• Authorization requests require professional time and may incur a fee if initiated without the required documentation.
• We only pursue authorizations when preferred medications are not covered, and a clinical alternative is needed.

📝 Forms & Pre-Op Clearance
Forms are completed only at Dr. Mkono’s discretion and must be discussed at check-in, not during the visit.
• Surgical clearance is not guaranteed.
• Submission does not guarantee approval by third parties.
• Wording must come directly from the requesting provider.
• Administrative forms (FMLA, DMV, etc.) may incur processing fees.
Please bring surgical details and contact information for the requesting provider to help us process your forms efficiently.

📂 Records & What to Bring
Bring the following to every visit:
• All inhalers or respiratory devices (even expired)
• CPAP machine or oxygen equipment
• Updated medication list
• Chest imaging discs (CT scans, X-rays)
• Insurance formulary list

Dr. Mkono can view images remotely from:
• Bethesda Health
• Delray Hospital
• Rayus Imaging
For other facilities, bring the image disc to your appointment.
Comparisons must be completed by your radiologist. If prior imaging is available, bring it to your imaging center for official comparison.

• Results are not provided over the phone.
• You must schedule a visit (in-person or virtual) to review:
• Breathing tests
• Lab results
• Imaging
• Post-procedure findings
If you are not contacted, you are responsible for booking a follow-up.
If preferred, you may review results with your primary care provider.

Records may be requested via our secure patient portal—please contact virtual staff for access.

• Only records created by Dr. Mkono’s office will be released.
• For outside records, contact the original provider.
• You may also request records via your primary care physician, if on file.
💰 Printed records cost $1.00 per page, plus mailing and administrative fees.
📠 We can fax your records to your PCP upon request (Dr. Mkono’s records only).
